THYROID
Hashimoto's Root-Cause Framework
  1. 1Get a correct diagnosis with a full thyroid panel, including antibodies (Ask your physician for a full panel including TPO and thyroglobulin (Tg) antibodies, not just TSH, to confirm a Hashimoto's diagnosis)
  2. 2Optimize thyroid medication with your physician (Work with your prescribing physician to find and adjust the right thyroid medication and dose for you; never start, stop, or change thyroid medication on your own)
  3. 3Trial a food-based elimination diet (A structured elimination diet trial to identify individual food triggers, ideally with dietitian support, since Wentz reports diet as one lever for symptom management)

THYROID
Persistent Hypothyroid Symptoms: When to Ask About Combination T3/T4
  1. 1Confirm your levothyroxine dose is adequate (commonly at least 1.2 mcg/kg/day) and rule out other causes of fatigue or brain fog before considering any change (Per your endocrinologist's dosing and labs)
  2. 2If symptoms persist despite biochemical euthyroidism, or your free T3 runs low on T4 alone, ask your endocrinologist specifically about a supervised levothyroxine/liothyronine combination trial (Prescriber-directed only; never self-dosed)
  3. 3Some endocrinologists use selenium 200mcg/day as an adjunct in autoimmune (Hashimoto's) thyroiditis (200mcg/day; check with your doctor, especially if pregnant)
What to buy
Selenium 200mcg ~$10

MENOPAUSE
Hormone Balance Protocol
  1. 1Get the right bloodwork (Cortisol (and its daily rhythm), a full thyroid panel (TSH, free T3, free T4, antibodies), sex hormones (estradiol, progesterone, testosterone), and consider a stool/microbiome test and a CGM)
  2. 2Eat for your hormones (Protein at each meal, plenty of cruciferous vegetables (broccoli, cauliflower, Brussels sprouts) for estrogen metabolism, fiber for the gut, and less sugar, alcohol and processed dairy)
  3. 3Dial down the stress load (Paced breathing: 5 second inhale, 10 second hold, 5 second exhale, 20 min twice daily; protect sleep; limit caffeine and alcohol)

  2. 2Fuel before training (About 15g protein pre-strength (a coffee plus protein shake works); add ~30g carbs if doing cardio up to an hour)
