Persistent Hypothyroid Symptoms: When to Ask About Combination T3/T4

Brittany Henderson, MD, ECNU, founder of the Charleston Thyroid Center, walks through the 2021 ATA/BTA/ETA joint consensus on combination levothyroxine (T4) plus liothyronine (T3) therapy: confirm adequate T4 dosing and rule out other causes first, then, if symptoms persist despite normal labs, ask your endocrinologist specifically about a supervised T4/T3 trial. This is prescription-only and physician-directed; the page never recommends self-dosing.

The protocol
Confirm adequate levothyroxine dosing first
Confirm your levothyroxine dose is adequate (commonly at least 1.2 mcg/kg/day) and rule out other causes of fatigue or brain fog before considering any change Per your endocrinologist's dosing and labs

Many persistent symptoms trace back to under-dosing or an unrelated cause, not a T4-to-T3 conversion problem; ruling this out first avoids an unnecessary medication change.

Ask about a supervised T4/T3 combination trial
If symptoms persist despite biochemical euthyroidism, or your free T3 runs low on T4 alone, ask your endocrinologist specifically about a supervised levothyroxine/liothyronine combination trial Prescriber-directed only; never self-dosed

The 2021 consensus names this as a considered option for a specific subgroup, not a default upgrade from T4 alone.

⚠ Liothyronine (T3) is a prescription medication. This page never recommends self-dosing, sourcing T3 outside a prescriber, or stopping levothyroxine on your own.
Selenium as an adjunct, thin-to-moderate evidence
Some endocrinologists use selenium 200mcg/day as an adjunct in autoimmune (Hashimoto's) thyroiditis 200mcg/day; check with your doctor, especially if pregnant

Small trials show selenium can modestly reduce thyroid peroxidase antibody titers; its effect on symptoms is not established, so it is graded honestly as a thin-evidence adjunct, not a core treatment.

Editorial, small-trial evidence flag
In your stack: Selenium
Retest 6-8 weeks after any change
Recheck thyroid labs 6-8 weeks after any dose or medication change, standard endocrine practice 6-8 weeks post-change

Thyroid hormone levels take weeks to fully reflect a dose change; testing too early gives a misleading reading.

2021 ATA/BTA/ETA joint consensus statement
The evidence 3

Most people on levothyroxine (T4 alone) feel well because the body converts T4 to T3 as needed.

Is this for you
  • People on levothyroxine whose symptoms persist despite a normal TSH
  • Anyone curious whether combination T3/T4 therapy is worth asking their endocrinologist about
  • Not for anyone considering stopping or self-adjusting thyroid medication without a prescriber
Cautions
  • Informational only, alongside your endocrinologist's care, not a substitute for it
  • Liothyronine (T3) is prescription-only; never self-dose or source it outside a prescriber
  • Disclosed conflict of interest: Dr. Henderson runs a private thyroid-focused practice and sells consults/telehealth through it
