MCAS: The Low-Histamine Diet and Trigger Diary

A companion to Dr. Lawrence Afrin's medication-focused step-up approach, this page covers the diet and tracking half: a 2-week symptom-and-food diary before changing anything, a 2-4 week low-histamine elimination trial, then reintroducing foods one at a time to find your real triggers rather than staying permanently restrictive. Evidence is honestly graded C: many patients report benefit, this is not proven in controlled trials.

The protocol
Keep a 2-week symptom-and-food diary
Log what you eat and any symptoms (flushing, hives, GI upset, lightheadedness) for 2 weeks before changing your diet Daily for 2 weeks

Identifies your actual pattern before you restrict anything, so triggers are found, not guessed.

Trial a low-histamine elimination phase
Avoid common high-histamine or histamine-releasing foods (aged cheese, cured/processed meats, fermented foods, alcohol, leftovers, certain fish) for 2-4 weeks 2-4 weeks, time-boxed

A defined trial window tests whether these categories matter for you specifically, without committing to indefinite restriction.

Reintroduce one food at a time
Add back one eliminated food every few days and log any reaction, rather than reintroducing everything at once One food every 2-3 days

This is what actually identifies your personal triggers instead of leaving you on a permanently restrictive diet by default.

DAO enzyme, a common patient-tried adjunct
Some patients try a DAO enzyme supplement before higher-histamine meals Per label, before meals

Evidence is preliminary (C-tier) and histamine intolerance is itself a contested diagnosis outside strict criteria. This is not part of Dr. Afrin's own published protocol; it is graded separately and honestly here.

Elimination-then-reintroduction is the standard structure for identifying food triggers in any condition where symptoms are variable and multi-factorial.

Is this for you
  • Anyone following the afrin-mcas-step-up medication approach who wants the diet half
  • People whose flushing, hives or GI symptoms seem food-triggered but who haven't identified which foods
  • Not a weight-loss diet, and not meant to be permanently restrictive
Cautions
  • Informational only, alongside your physician's care
  • Not a weight-loss diet; overly restrictive long-term elimination without reintroduction risks nutritional gaps
  • MCAS itself is a genuinely contested diagnosis outside stricter Consensus-2 criteria
  • If you have any history of disordered eating, do this elimination trial only with professional support, and consider skipping the restriction phase
  • Educational only, not medical advice
