What it is

Tadalafil is a PDE5 inhibitor, the same class as the ED drug Cialis, that relaxes blood vessels and improves blood flow. Bryan Johnson takes 2.5mg daily and frames it as longevity medicine because of large datasets linking it to lower mortality and disease. This is worth taking seriously and reading carefully. The headline numbers come from one big observational study (Jehle et al., 2024), and observational means association, not proof: the men taking tadalafil were prescribed it for erectile dysfunction or prostate symptoms, and that group tends to be healthier and more engaged with healthcare than those not prescribed it. No randomized trial has shown tadalafil extends life or prevents dementia in healthy people. It is also a prescription drug with real contraindications. So this page documents what Johnson does and the evidence behind it; it is not a recommendation to start, and there is nothing to buy here.

Why it worksâ–¼
PDE5 inhibitors cause vasodilation (wider blood vessels, better flow), which is the mechanism behind both erections and the proposed cardiovascular and brain benefits. In Jehle et al. (2024), an analysis of around 50 million US men, those prescribed tadalafil after an ED or prostate diagnosis had lower three-year rates of death (about a third lower), heart attack, stroke and dementia than men not prescribed it. Those are the numbers in Johnson's post, and they are real, but they come with heavy confounding: this is a comparison between people who were and were not prescribed a drug, not a clean test of the drug itself. Mechanistic plausibility plus association is interesting; it is not the same as proof, and authoritative reviews note no trial has tested longevity or dementia prevention in healthy users.
